How to use this page

  1. Below, you will find two online survey forms identical to the survey forms you have/will be administering.
  2. Input the data from each of the surveys in the correct form (community member form is first. Scroll down for CHW/TBA/Pharmacists form)
  3. Click submit
  4. Click submit another response and repeat for all survey forms.
  5. Check on the Analysis page or on the email-linked excel form to see that all surveys have been input accurately.
